Action for Neighbourhood Change (ANC)

Breastfeeding Support
- Counselling for breastfeeding mothers
- Phone calls or home visits
- Breast pump rentals
- Discussion of the effects of foods and medications on breast milk
Chronic Disease Self-Management
- Sessions to learn ongoing self-care includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions
Clinical Dietitian
- Nutritional information and education
- One-on-one, couple, or family sessions for people with food and health-related concerns
Diabetes Education
- Information about living with diabetes including monitoring, blood sugar levels, food choices, exercise, medications, and lifestyle choices
Fitness and Recreation
- Exercise to strengthen muscles, the heart, and the lungs
- Gentle programs for older adults
Foot Care Clinic
- The chiropodist (foot care specialist) assesses and treats common foot problems includes basic foot care for both regular clients and those living with diabetes
Health Promotion
- Services range from group programs to community presentations and participation
- Focus on priority populations including youth, older adults, new immigrants, people affected by mental health issues, people with multiple complex chronic diseases, and under-resourced parents
- Free child care and interpretation is provided for most programs
- Open to everyone with a focus on priority populations
Health Services
- The health care team includes family doctors, nurse practitioners, nurses, social workers, case coordinators, dietitians, and chiropodists (foot care specialists)
Mental Health Counselling
- One-on-one, couple, and family counselling
- Mental health screening and assessments
- Short-term and long-term counselling
- Services are confidential
Seniors' (Older Adults) Drop In
- Companionship and shared activities
- Information sessions on health and social issues
Older Adults' Health Care
- The aim is to help seniors remain independent in the community for as long as possible through counselling, support, education, and referral to other services including counselling, needs assessment, case management, advocacy, outreach, and coordinating services into the home.
Smoking Cessation
- One-on-one support to help quit smoking
